General Release and Waiver of Liability

PLEASE READ CAREFULLY. This is a legal document that affects your legal rights.

By checking the box below, you acknowledge that you have read, understand, and agree to the terms of this General Release and Waiver of Liability (“Release and Waiver”), executed by registered participant in favor of Parent Heart Watch (PHW), a non-profit organization, its directors, officers, employees and agents. The registered attendee hereby freely executes this Release and Waiver under the following terms:

1. Waiver and Release. The attendee releases and forever discharges and holds harmless PHW and its successors and assigns from all liability, claims and demands of whatever nature, either in law or in equity, which may arise from the attendee's participation in the conference, related events, and activities, (collectively, the "Activities"). The attendee understands and acknowledges that this Release and Waiver discharges PHW from any liability or claim that the attendee may have against PHW with respect to any bodily injury, personal injury, illness, death or property damage that may result from attending the conference.

2. Medical Treatment. Except as otherwise agreed to by PHW in writing, the attendee hereby releases and forever discharges PHW from any claims whatsoever which arise on account of any first aid, treatment or service rendered in connection with the attendee's participation in the Activities.

3. Insurance. PHW does not assume any responsibility for or obligation to carry or maintain health, medical or disability insurance coverage for any attendee.

4. Indemnification. The attendee hereby agrees to indemnify and hold harmless PHW from any loss, liability, damage or cost he or she may incur arising out of or related to the conference, whether caused by the negligence of PHW or otherwise (but not including intentional acts of PHW).

5. Applicable Law. This Release and Waiver shall be governed by the laws of the State of Ohio.

6. Photo Release. I hereby authorize PHW, its agents, successors, subsidiaries, and/or affiliates to use/publish my image/photo in brochures, newsletters, websites and/or miscellaneous publications without limitation. Said photo(s) shall be and remain the property of PHW. Said photos(s) may be used with or without using my name. 

7. Dietary Needs & Allergies. Parent Heart Watch is not responsible for adverse reactions due to food allergies or dietary restrictions. Attendees with severe allergies should bring any necessary medications (e.g., EpiPen) and exercise caution when consuming meals provided.
